I though my mapper 90 was perfect... until this time.

Can FCEUX run this game? Filename + MD5 checksum (or CRC32 checksum)? If FCEUX can run it, then it might be possible to see what's going on through RE efforts.
NO$NES or Nintendulator are other possibilities, assuming they have support for mapper 90 (unsure).

Mirroring control $D000 should always work & disable nametable control.
A few games will break if nametable control is active.
It's solved, but must be a manual setting, since I'm not discussing mapper number anyways.

No. I mean registers $D00x (except $D001) enable nametable control (NC). This game (MKII and variants) ignore NC, only $D001 mirroring control should work. Other games (Samurai Spirits?) break if you disable $D00x.
